Our Financial Assistance Policy applies to Missouri residents and includes services provided at WCMH and affiliated clinics.

1. Obtain and complete the Financial Assistance / Sliding Scale application (available from billing or online).
2. Provide documentation, such as:
   
   • Income (past 3 months or last year tax return)
   • Proof of residency
   • Household size, savings, assets, expenses
   • Medicaid denial or pending application (if applicable)
3. Submit it to the WCMH Business Office within 10 days of the service or appointment.
4. Applications must be renewed every 12 months.
5. Your account will be reviewed, and you will be notified via mail of eligibility and discount levels.

• Primary Care Clinic Visits: A reduced rate may apply for patients who qualify under the sliding scale.
• Specialist / Other Clinic Visits: A reduced visit rate may apply under the sliding-scale policy.
• Sliding scale for clinics is separate from hospital billing, please complete application and turn into clinic were services are taking place.
• Note: Chiropractic services do not qualify under the sliding scale.

Discounts apply only after submission of a complete financial assistance application.
