At WCMH, we believe managing chronic conditions should feel supportive—not overwhelming. Our Chronic Care Management (CCM) program is designed to help patients with multiple ongoing health conditions stay healthier, more connected, and better supported between office visits.

Who Is Eligible for CCM?

  • Have two or more chronic conditions expected to last at least 12 months (or for the rest of your life) that require ongoing monitoring, coordination, and support
  • You have received a referral from your physician and provide consent to participate, ensuring your care team can coordinate services and support your health goals
  • Have Medicare Part B coverage, which allows you to receive CCM services. A monthly fee may apply, and the Medicare Part B deductible and coinsurance are required. Patients with supplemental insurance or dual Medicare and Medicaid coverage may have some or all of these costs covered.

What We Do

  • Collaborate with your healthcare team (doctors, nurses, therapists, pharmacy) and specialists to create and monitor your care plan.
  • Monitor symptoms and help manage medications and treatment plans.
  • Help reduced unnecessary readmission with chronic condition management, wellness education, and transitions to help promote recovery
  • Answer health-related questions and address concerns without requiring an office visit.
