What We Do
- Collaborate with your healthcare team (doctors, nurses, therapists, pharmacy) to create and monitor your care plan.
- Assist with discharge planning — making sure your next steps are clear, your medicines are understood, and follow-up is arranged.
- Help identify and connect you with community resources, home health services, rehabilitation, and post-acute care if needed.
- Support chronic condition management, wellness education, and transitions to help prevent readmission and promote recovery.
Your Role & How We Support You
- You’re encouraged to ask questions: about your care plan, medications, follow-up appointments and what to watch out for at home.
- We’ll provide you with written materials, contact lists, and support resources tailored to your needs.
- If you’re unsure about what happens next, our Care Management team is available to guide you and your family.
- Together we aim for a smoother recovery, fewer surprises, and a safe transition from hospital to home or other care settings.
